Summer Camp 2026 Leader Medical Form

Thanks for volunteering for Summer Camps 2026!

Cost: €30 per leader. By giving a leader contribution, it helps keep the cost down for the campers. We appreciate leaders giving of their time and understand that each person has a different financial situation. We do not want finances to be a problem so please give what you are able. If you want to give more or less than €30, use the donation box option on the form to enter the amount you can give.

***If you are leading at multiple camps, add multiple tickets for the number of camps you are doing. Example: If you are volunteering for 2 camps, select 2 tickets.
