Older Adult Hospital at Home Stakeholder Awareness Session

Find out about our new Hospital at Home service for older adults with mental health needs.

This awareness session offers an opportunity to learn about this new service that will soon become part of the older adult mental health pathway across the Black Country. Hospital at Home will provide intensive, specialist support in people’s own homes as an alternative to hospital admission where appropriate, as well as supporting earlier discharge from inpatient care.

Join us to hear about the service model, the benefits for patients, carers and professionals, and what the introduction of Hospital at Home will mean for local services and partnerships. Whether you work in healthcare, social care, the voluntary sector or a partner organisation, this session will help you understand how the service will support more personalised, recovery-focused care and improve outcomes for older adults across the Black Country.
