Home & Well with Citizens Advice

Home and Well is a unique partnership of organisations working together to help residents across Hampshire and the Isle of Wight return home safely, quickly, and comfortably after a hospital stay. The service focuses not only on supporting recovery at home, but also on creating a warm, safe, and suitable living environment that promotes long-term wellbeing and independence.

The project brings together support for fuel and water poverty, identifies Priority Services Register (PSR) needs, and takes a holistic approach to wellbeing by connecting people with services that can help with wider challenges, including housing, employment, finances, and relationships.
