This online session allows participants to watch a real time interactive demonstration led by EC4H tutors. Participants will have the opportunity throughout the demonstration to contribute suggestions and feedback to the tutor team via the chat box.

Scenario

Mrs Jones is an 80 year old care home resident with frailty / multi morbidity and a possible urine infection.   She was admitted to hospital  yesterday evening following an assessment by an out of hours GP.  Mrs Jones has been given IV fluids and a dose of IV antibiotics.  The drip tissued this morning and has not been replaced as access is very difficult and patient is able to take oral fluids and medications.  The Consultant this morning decided the patient is stable enough to return to the care home with continued oral antibiotic.  A DNA CPR form was completed on admission.  The care home is happy to take over care and an ambulance has been booked for this afternoon.

The patient's daughter and next of kin is very angry and upset about her mothers care.  She has asked to speak to the Consultant but they are not available until later this evening.  The senior health professional meets with the patients daughter to address her concerns

Joan currently works as an Advanced Care Practitioner in Resuscitation and Clinical Risk within NHS Western Isles. She has been an adult trained nurse for 21yrs, more recently working in resuscitation. Joan has been an EC4H instructor since September 2021 and is the Lead Tutor for NHS Western Isles.

Donna is a cancer and palliative clinical nurse specialist in the Western isles. She has been involved in promoting the importance of effective communication in advanced care planning and also the education of student nurses. More recently she completed her tutor training with EC4H in March 2022.
