Together with our London Regional Hub Guy's & St Thomas' NHS Foundation Trust (GSTT), we are hosting a Showcase event on Simulating Success: Integrating Human Factors for Enhanced Patient Safety.

Agenda:
Welcome - Jon Wilks, CEO, IHSCM
12:30-12:55pm Cindy Sethi, Consultant Physician in sexual health and HIV, Deputy Chief Medical officer, Patient Safety and Clinical Effectiveness will join us to discuss their Patient Safety Incident Response Framework (PSIRF) and learning responses.
This session will help you understand the PSIRF by:
- Describing the key components and principles of the Patient Safety Incident Response Framework
- Explaining the purpose and indications of different learning responses
- Appreciating the importance of embedding a culture of safety and continuous improvement in healthcare
Cindy Sethi is a consultant physician specialising in sexual health and HIV with over 20 years of experience. Renowned for a deep commitment to quality improvement and patient safety, she currently serves as the Deputy Chief Medical Officer for Patient Safety. In this role, she leads the implementation of the Patient Safety Incident Response Framework and supports the priority patient safety improvement plans within the Trust. She is a passionate educator with a passion for advancing healthcare standards and fostering a culture of safety and excellence in patient care.
12:55-1:20pm - Paul Greig, Consultant Anaesthetist, Guy's and St Thomas' NHS Foundation Trust, Senior Clinical Research Fellow, Nuffield Department of Clinical Neurosciences, University of Oxford will join us to address human factors through the prism of critical incidents.
After this session, you'll be equipped with understanding about:
- Defining the term ‘human factors’ in the context of critical incidents
- Using human factors principles to explore a real critical incident
- Concepts including 'production pressure' and 'bystander effects'
Paul Greig is a consultant anaesthetist at Guy's and St Thomas' NHS Foundation Trust. He is the directorate lead for clinical governance, and has an interested in human factors and patient safety. Paul is also a senior clinical research fellow with the Nuffield Department of Clinical Neurosciences, University of Oxford, is a member of the Resuscitation Council (UK)'s ALS subcommittee, and teaches non-technical skills at the Royal College of Anaesthetists. He has advised HM Prison and Probation Service on operational culture and the systems approach to critical incident investigation
1:20-1:45pm Colette Laws-Chapman, Lead Nurse for Non-Medical simulation programmes and Patient Safety Simulation at GSTT and President of the Association for Simulated Practice in Healthcare (ASPiH) will join us to address Patient Safety through the lens of Transformative Simulation.
By the end of this session, you'll have gained knowledge on:
- The term ‘Transformative Simulation’ in the context of simulation for non-educational purposes
- The principles of simulation based innovation to explore new environments
- How simulation based involvement can capture living memory when reviewing rea-life incidents
Colette Laws-Chapman is the Lead Nurse for Non-Medical simulation programmes and Patient Safety Simulation at GSTT and President of the Association for Simulated Practice in Healthcare (ASPiH). Colette is a Registered nurse with an intensive care specialty background, is passionate about education, training, and quality improvement, having worked within Simulation and human factors since 2009 she describes herself as a simulation specialist. Colette is the first nurse to become president of ASPiH and is set to accompany the association in the celebration of its upcoming 15th anniversary in Nov. 2024.
