Working Together to Improve Transitions into Adulthood

We are delighted to invite colleagues and partners from across the East of England to join the new ADASS & Improvement East Preparation for Adulthood (Transitions) Community of Practice.

This Community of Practice has been established to bring together professionals from across children's and adults' services, health, education and partner organisations to share learning, strengthen collaboration, and drive improvements in the experiences and outcomes of young people as they transition into adulthood.

Join Our First Community of Practice Meeting for an opportunity to:

  • Hear about progress made across the region over the last year
  • Receive updates on national developments and priorities
  • Learn from examples of effective practice
  • Share experiences, challenges and solutions
  • Build connections and strengthen collaboration
  • Help shape future regional priorities and work programmes

Background and Progress to Date

Since July 2025, leaders and practitioners from children's and adults' services across the Eastern Region have been working together to strengthen transition arrangements and improve outcomes for young people moving into adulthood.

Key milestones have included:

  • The ADASS & ADCS Transitions Event (July 2025)
  • Regional Mental Capacity Act (MCA) and Deprivation of Liberty Safeguards (DoLS) Training
  • The Eastern Region Transitions Conference on Transitional Safeguarding (February 2026)
  • A regional diagnostic review involving all 12 local authorities across the East of England

The diagnostic review identified significant variation in approaches and practice across the region. One of its key recommendations was the development of a Regional Community of Practice to promote collaboration, share learning, and support continuous improvement in transition services to enhance the experiences and outcomes of children, young people and their families through stronger regional partnerships and shared best practice.
