LOTI Sandbox Design Session: Transfer of Care
About the event
Following the LOTI Adult Social Care Immersive Day on 3 October, we invite you to a series of design sprints where your insights, ideas, and experiences will shape tangible solutions. This in-person session focuses on the Transfer of Care, exploring how we can improve the process for both patients and staff, ensuring smoother transitions, better information sharing, and appropriate support.
Join us for this interactive session where we will:
- Review the challenges individuals and staff face during care transfers, including sharing information and managing assessment forms.
- Explore ways to enhance patient agency and ensure they receive the right support
- Collaboratively generate and refine solutions, kicking off the next phase of the process
If you have ideas or perspectives on how care transfers can be streamlined and better designed to support both patients and staff, this is your chance to contribute and work alongside others to shape practical solutions.
Who should attend?
- Hospital staff or social workers involved in discharge and transfer of care
- Staff working in transfer of care hubs
- Trusted assessors or care home staff conducting pre-admission assessments
- Individuals with lived experience
