Join us for a special community event as Health Partners releases the latest 2025-2026 Sandusky County Community Health Assessment, offering a comprehensive look at the health of our community. This important report highlights key data, trends, and insights that help guide local efforts to improve health and well-being for everyone.

Following the presentation of the assessment, we will also celebrate a major milestone — 30 years of the Prevention Partnership Coalition. For three decades, the coalition has brought together community partners, organizations, schools, and leaders committed to preventing substance misuse, promoting healthy choices, and strengthening our community.

This event is an opportunity to learn about the current state of our community’s health, recognize the partnerships that have made lasting change possible, and celebrate the collaborative work that continues to move us forward.
