We are all challenged by COVID-19. We are all in this together and we need your help to ensure we can return to services in a safe manner. Thank you for your patience and understanding during this transitionary phased re-opening period for TBS-EV.
Attending in-person events is considered a high-risk activity for contracting COVID-19 and other illnesses. Although we have put mitigation measures in place to help lower the risk of spreading illness, it is impossible to fully eliminate the risk. We strongly encourage all attendees to be fully vaccinated when possible. By attending in-person events and services, you acknowledge and assume your own risk and agree to abide by the policy in place.
PLEASE READ ALL OF THE INFORMATION BELOW and acknowledge for all people you are registering. These requirements are being updated by the board periodically so the only way to ensure you understand the current requirements is to read through them each time you register.
I acknowledge that:
During medium and high risk levels:
-I understand that I MUST maintain social distancing of a minimum of 6 ft. separation from all of those outside of my social pod at all times
-I understand that everyone I register MUST wear a mask that properly covers the nose, mouth, and chin following CDC recommendations, for indoors only for medium risk and indoors & outdoors for high risk. CDC Resources: Types of Masks, About Face Coverings
-I understand that the capacity in the Sanctuary is currently limited by the social distancing requirement. This includes Bar/Bat Mitzvah events and by submitting this form I have full intention to attend as submitted.
At any time, including during low, medium and high risk levels:
-I do not have a fever. I feel healthy, and I have had no flu-like symptoms or other symptoms consistent with COVID-19 infection for at least seven days.
-I have not had any loss of the senses of smell or taste in the past seven days.
-I do not have shortness of breath, cough, runny nose, congestion, or other respiratory symptoms and haven’t in the past seven days.
-I have not had close or sustained contact with anyone with the above symptoms in the past 14 days.
-I understand that if I do not follow the practices listed above that I will be asked to leave the premises immediately without question.
-I will notify the event organizer or president@tbsev.org immediately if I, or one of my guests, tests positive for COVID-19 within 7 days of an in-person event.
Please be respectful to other congregants at all times, and their personal choice to continue to wear a mask and practice social distancing if they so desire. If you cannot make it to the event or would like to be removed, PLEASE use the “CONTACT THE ORGANIZER” button below to cancel your reservation(s) and free up the space for others. If you think you made a ticket please check with us or use the lost ticket option.
