Please complete this Electronic Enrolment Form FOR THE CHILDREN ATTENDING UNIYOUTH. Please complete one separate form per child. A SEPARATE ENROLMENT FORM FOR THE ADULTS will be sent.

We warmly welcome you and your children to our newly formed UniYouth weekend Fri-Sun, 2nd-4th October 2026 being held at The Nightingale Centre, Great Hucklow https://www.thenightingalecentre.org.uk/. UniYouth is a youth weekend that welcomes children of ages, 7-14yo, to the centre together, rather than the separate weekends for Juniors and Inters as in the past. All children will share The Nightingale Centre with meals, excursions and worship, but we will have separate programmes for two age groups: 7-11yo's with their parents/carers and unaccompanied 11-14yo's, who will also be housed in a separate wing.

ALL UNDER 18'S ARE FREE OF CHARGE FOR FULL BOARD, although Tickets and Coach trip to Jodrell Bank is only partly subsidised by Unitarian College, so we kindly request £30/child for the trip. Please note payment has to be made separately from this electronic enrolment form, via BACS:

Consent

Whenever medical advice or treatment is needed, the assistance of a GP or A&E Department of a hospital should be sought. The Children Act 1989 allows a doctor to provide any necessary treatment by doing ‘what is reasonable in all the circumstances of the case for the purpose of safeguarding or promoting the child’s welfare’.

However, the parent/carer should be contacted and advised of the situation as soon as possible. It is important, however that those caring for children and young people on day trips, outings and residential activities obtain in advance from the parent/carer:

1. All necessary information concerning the child/young person’s health, allergies, medication etc.

2. Written agreement as follows:

I understand:

· My child will receive medication as instructed before or during the event.

· Every effort will be made to contact me as soon as possible should my child become ill or have an accident.

· My child will be given medical/dental treatment as necessary.

The information requested on this form can only be completed by those with parental responsibility. (NB This may not include a foster carer).

This form will be kept securely by one of the youth leaders and brought along to the event.
